Create a "Something Broken? Report It" link in Primo

Hi everyone,

I updated these instructions.  I noticed that the add-on I was using for email notifications was no longer working.  So I switched the add-on to a different one published by Google Cloud.  It doesn't send you the form fields in the email like the other one, but it notifies you when someone submits the form.  You can then login and view their responses.
