EBSCOhost Search Box Builder

Does anyone know the appropriate way to complete the EBSCOhost Search Box Builder?  It's been a while since we've created any and we're doing some updating.  The part we are getting stuck on is the Authentication Parameters in Step 2 shown on the form or in the screenshot attached, does anyone know what they're looking for in the "proxy prefix" or "methods" selections?  Any help is appreciated!  Thanks in advance!

Each college has its own EZProxy instance. I don't know what code Emily used for LFCC when creating your proxy, but the format is http://ezxxx.vccs.edu:2048/login?url= where you replace the xxx in ezxxx with your code, perhaps, ezlfc?

As for 'method,' I think that should be IP Address. j
